Scott Ogley, M.D. and the Guerra Plastic Surgery Center respect and protect the privacy of those who visit or use our Web site; we want you to know how your information is used. To demonstrate our commitment to fair information practices, we have adopted privacy guidelines accepted by industry leaders. Our on-line privacy policy protects the information that you share with us over the Internet. We employ industry-recognized safeguards to protect the personally identifiable information that you have provided to us against loss, misuse, and unauthorized alteration.

When using our site you may get information or communicate with us. To gain a better understanding of people like you who are interested in our work, we may request information from you when you visit our Web site. Excluding financial planning information, this information is never tied to any personally identifiable information.

Do Not Track Requests

What Information is Collected and How is it Used?

Contact Form

We collect contact information such as name, email address, and phone number. We use this information to contact you in the case that you have specified via the “contact form” on our web site that you are interested in making an initial consultation at our office. This information may go to our leads partner Elite Med Listings and be reviewed for first contact with you in order to respond directly to your questions or comments. We may also file your comments to improve the site, or review and discard the information.

Information in the form is also collected by P5 marketing for leads generation purposes.

Phone Calls and SMS Texting

When you dial our phone number, the call may connect with our leads partner Elite Med Listings. Any information shared with them is stored for future contact with you only. This information is shared to us, and no other party.

When you send a SMS Text to our number, it may be handled by Elite Med Listings, and shared to us. This information is not shared to any other party.

Any messages you receive from our SMS Text number are either replies to contact from you (via Contact Form, email, phone call, SMS text), or information you subscribed to such as appointment reminders. We do not engage in mass SMS marketing.

Analytics and Cookies

We use Google Analytics, and Google AdWords to track user activity and engagement to the website. This data does not allow us to identify data pertaining to individual users. This data is shared with PrimeView and P5 marketing to evaluate performance of the website and various promotions.

We use a first-party cookie solely for consent to view our photo gallery. This cookie does not contain any personal information.

Tracking Pixel

We embed a tracking pixel on every page of this website to track your activity while on our website. Website activity including contact form data and pages visited is collected by P5 marketing and subject to their privacy policy. This technology enables anonymous visitor identification. They share lead data to us such as name, addresses, phone number, email address, and IP address. We use this information to contact potential patients who have not initiated contact with us.

As per their privacy policy, they do not sell your information to other parties.

Security of Collected Information

We maintain strict physical, electronic, and administrative safeguards to protect your personal information from unauthorized or inappropriate access.

Any person or entity viewing these pages is a “user/s” as that term is used throughout this statement of site terms & conditions. Users must agree with, and agree to abide by, each and every provision contained herein in order to use this site. Read this agreement carefully. SO reserves the right to change, modify, and update this site and reserves the right to amend these terms & conditions without prior notice to users. Users agree to periodically review these terms & conditions to ensure that each agrees to continue to agree with, and abide by, these terms & conditions. SO provides this site as an informational service to our patients and members of the community to inform and educate them about plastic surgery procedures and the plastic, reconstructive & aesthetic surgery provided by SO.   7. Venue and Jurisdiction. This Agreement is entered into in Phoenix, AZ. You agree that Arizona State law (regardless of conflicts of laws principles) shall govern this Agreement, and that any dispute arising out of or pertaining to this Agreement shall be subject to the federal and state courts in Phoenix, AZ. You consent to the jurisdiction of the State and Federal courts in Phoenix, AZ. You agree that venue for any and all disputes arising out of or pertaining to this agreement, or relating to SO shall be in the appropriate State or Federal Court in Phoenix, AZ.
  8. Attorneys’ Fees. In the event either SO or user employs an attorney to enforce or defend any claim arising out of this Agreement, the prevailing party shall be entitled to recover from the losing party, and the losing party shall pay all reasonable attorney fees and costs incurred by the prevailing party, whether in negotiation, pre-trial, trial, or on appeal.

If you previously had a FREE consultation then a $100 non-refundable fee for a second consultation is required.

We require a $25 scheduling fee for all non-surgical procedures. Two (2) business day notice is required if you need to cancel or reschedule your appointment to avoid any fees.
